Step by step directions on HOW TO COMPLETE FORMS:
STEP 1 - INFORMATION NEEDED TO COMPLETE FORMS:
The parent/guardian will be asked to provide information:
• Name and contact of primary physician
• Emergency contact phone numbers
• Insurance policy information (if possible)
STEP 2 - TO ACCESS THE REQUIRED ATHLETIC FORMS:
Click on the “HIGH SCHOOL FORMS” or “MIDDLE SCHOOL FORMS” tab. You will get a drop down list of all required forms.
STEP 3 - TO BEGIN TO COMPLETE FORMS:
Click on the form name and fill out the information requested. Parent/Guardian must have their son/daughter present to complete forms. If you do not know the answer to a question, please answer the question with “N/A” or “none”.
STEP 4 - TO SIGN THE FORMS:
• Click inside the signature box and hold your mouse down, this will allow you to create an “electronic signature”.
• If you make a mistake and need to start over, click on the refresh icon next to the signature box.
• If you are using an IPAD, you may create an electronic signature by holding your finger to the screen and signing your name or by using a stylus.
STEP 5 – Repeat STEPS 1 – 4 for all required forms shown
TO PRINT COMPLETED FORMS (FOR YOUR RECORDS only):
Once you have filled out all the information on a form, you will have the option to print the completed form for your records if you wish. You will also receive an email confirmation that you can print.
REMINDERS:
• To submit the forms online appropriately, you and your child must sign in all required spaces. Forms will not be accepted electronically without all required information being completed.
• You will receive a confirmation email once the forms are submitted and approved by the Southwest Christian Athletic department.
• The TAPPS Physical Examination Form cannot be completed electronically because it requires a physician’s signature.
